Overview Health Homes Care Coordinator role at WNY Independent Living Inc. Salary $42,000 - $46,000/year.
Responsibilities Perform outreach and enrollment activities to assist consumers in the community; explain the benefits of Health Homes and enroll individuals; coordinate services to decrease high-risk medical and/or behavioral health costs for individuals served in Health Homes.
Identify areas within the region where people with high medical risks are encouraged to seek health care and conduct outreach and engagement in collaboration with Western NY Independent Living, Inc. (WNYIL).
Determine Medicaid eligibility through ePACES; if eligible, collaborate with Medicaid Application Assistance Program (MAAP/ABD) to certify/recertify as needed.
Coordinate WNYIL staff to meet established time benchmarks for Health Homes services and ensure follow-through on reducing barriers to keeping appointments.
Coordinate WNYIL direct service staff to collaborate with individuals, community and family supports to meet identified social needs.
Identify and recruit individuals who qualify for Medicaid and have two or more chronic health conditions, serious mental illness, HIV/AIDS, or significant risk factors that can be addressed with coordinated care (e.g., homelessness).
Facilitate and document face-to-face referrals from community providers; contact potential Health Homes enrollees via phone, letter and face-to-face contact.
Enter all required data into program reporting databases (including care plans, referrals and assessments) and maintain required records in both SPR and ECR systems within two working days of service.
Maintain neat, clean and safe work environment; participate in other job-related duties as assigned by the supervisor.
Education & Training / Qualifications Must possess a Bachelor’s degree in one of the following areas: Child & Family Studies, Community Mental Health, Counseling, Education, Nursing, Occupational Therapy, Psychology, Recreation, Recreation Therapy, Rehabilitation, Social Work, Sociology or Speech and Hearing.
OR NYS licensure and current registration as a Registered Nurse AND a bachelor’s degree; OR bachelor’s degree or higher in any field with five (5) years of experience working directly with persons with behavioral health diagnoses OR a CASAC.
Must have a Master’s degree in one of the listed fields, with one (1) year of experience; OR a Bachelor’s degree with two (2) years of experience in direct services to persons with mental disabilities, developmental disabilities, alcoholism or substance abuse.
Experience Relevant experience in coordinating health home services or related behavioral health support preferred.
