Starting Pay: $20/hr
Schedule: This is an hourly, full-time position, Monday through Friday, working 8 hours per day. Standard business hours are between 8:00 AM and 5:00 PM, with some flexibility in scheduling.
Job Summary:
The Health Home Care Manager is responsible for providing comprehensive Care Management services to specific populations including those in recovery from Physical, Mental Health and Social setbacks. The Care Manager fosters health promotion, transitional care, patient support and referral to community and social support services. The Care Manager is responsible for creating and executing a Care plan that links individuals to services in order to support the improvement of their health and wellness.
Qualifications:
Bachelor’s degree in Social Work or Human Services related degree preferred OR Associate’s degree in Human Services or related area with 3 years full time work experience or its part-time equivalent working with persons that were homeless or underserved OR High School Diploma with at least 5 years full time work experience or its part-time equivalent working with persons that were homeless or underserved will be considered.
A minimum of 2 years’ health home care management experience preferred.
Bilingual and able to read, write and speak Spanish proficiently preferred, but not required.
Possess social justice and human rights perspective and frameworks that align with agency's principles and mission.
Knowledge of community resources and social work practices with homeless or underserved populations.
Ability to work independently with strong focus, task oriented, non-judgmental and able to set clear boundaries.
Possess a strong sense of respect for confidentiality involving both participants and peers.
Possess an ability to work in a variety of settings with culturally diverse individuals and families, and remain sensitive to their individuality.
Knowledge of and/or informed practice of motivational interviewing, trauma informed care, etc.
Possession of a valid NYS driver’s license and a driver’s record considered acceptable by agency and insurance carrier and availability of a car for daily use including transporting clients to appointments.
Effective oral and written communication, ability to establish priorities, work independently, manage multiple tasks.
Excellent engagement, written and verbal communication skills and documentation skills.
Essential Functions:
Align with the vision of the agency while creating and maintaining an environment that encourages teamwork, collaboration, and a positive work environment.
Completes care plans for assigned clients and reviews and updates goals and assessments accordingly.
Participates in interdisciplinary meetings and treatment team meetings.
Utilizes health information technology to document services and record progress in a timely and thorough manner.
Assist clients in establishing and maintaining benefits and entitlements that foster health promotion.
Ensures responsible delivery of client care per established Care Management policies and procedures.
Participates in weekly health home team case conference meetings as well as bi-weekly one-on-one supervision with Supervisor.
Maintains updated knowledge of other existing community resources and utilizes appropriately.
