Coordinate and facilitate client care needs in behavioral and medical domains.
Collaborate with all relevant service providers to eliminate barriers to efficient care.
Effectively communicate to clients and family members regarding plan of care and progression through stages of care management.
Use health information technology to link services.
Adhere to hospital confidentiality statement.
Participate in Primary Care team meetings and coordinate care with Chronic Care Management Team.
Qualifications
Bachelor’s degree in an appropriate discipline preferred with one to two years progressive experience, or Associate’s degree with a minimum of four years experience acceptable.
Previous discharge planning, counseling, home care, and substance abuse treatment experience desired.
Organizational, management, and leadership skills; oral and written communication skills; problem solving and conflict management ability.
Pay Range
Hourly rate: $20.25 – $29.30
