Overview HCBS Case Manager – Remote in Minot, ND (Telecommute). The ND Department of Health & Human Services (HHS) Adult & Aging Services Division seeks a Home and Community Based Services Case Manager to assist individuals in staying in their own homes with supportive services. Travels to Ward and McLean Counties and surrounding areas may be required.
Responsibilities Provide case management to elderly and physically disabled individuals under the Home and Community Based Services Program.
Perform assessments, develop and implement individualized person-centered care plans, and provide monitoring and ongoing case management.
Make referrals to community partners and maintain records and documentation.
Qualifications Licensure as a Licensed Baccalaureate Social Worker (LBSW) by the ND Board of Social Work Examiners, or licensure as a Baccalaureate of Nursing and licensure for RN (BSN/RN) by the North Dakota Board of Nursing.
One year of professional human services work experience as a social worker, human relations counselor, developmental disabilities case manager, mental illness case manager, vocational rehabilitation counselor, activity therapist, addiction counselor, registered nurse, employment counselor, or a similar professional level position in the public or private sector.
Or a Master’s Degree in social work with licensure as a Licensed Social Worker by the ND Board of Social Work Examiners.
If no applicants meet the above qualifications, consideration may be given to applicants who have passed the ND Social Work licensure exam and are awaiting license number, or candidates who have LBSW with less than one year of experience.
Location: This position may work from home and travel to Ward and McLean Counties and surrounding area.
