Overview Gun Diversion Case Manager – Lower Eastern Shore at Lead4Life, Inc.
Location: Lower Eastern Shore, MD
Salary/Hourly Rate: $44,850 annual
Position Status: Full-time W-2
Reports To: Executive Director / Senior Director of Programs
Responsibilities Provide direct, high-touch case management to youth referred to the Gun Diversion Program, including intake, assessment, service coordination, and progress tracking.
Conduct comprehensive intake assessments to evaluate risk factors, strengths, and service needs; develop individualized service plans focused on goals, accountability, and reducing risk of future offenses.
Meet regularly (primarily in-person) with assigned youth for ongoing support, check-ins, and progress monitoring.
Maintain weekly communication with participants via phone, text, or home visits; assist with accessing education, employment, housing, behavioral health, and other supports.
Attend court hearings as a support for youth and report on progress to the court and prosecutors.
Track attendance and participation in program sessions and document progress in the tracking system; notify stakeholders weekly.
Maintain regular contact with Department of Juvenile Services, State’s Attorney’s Office, and probation/parole officers; provide timely updates to case workers and guardians.
Engage in restorative circles or victim reconciliation processes when appropriate; participate in team meetings, trainings, and community outreach.
Issue certificates upon successful program completion; attempt re-engagement for disengaged participants with regular follow-up for up to 30 days; document all re-engagement efforts and escalate non-compliance as needed.
Complete discharge summaries and notify referral sources of participant status; ensure data entry of case notes, attendance, and outcomes; maintain compliance with documentation standards; support grievance processes per agency policy.
Minimum Requirements Associate or bachelor’s degree in criminal justice, Social Work, Youth Development, Psychology, or related field (OR equivalent lived/professional experience).
At least 2 years of experience working with justice-involved or at-risk youth, ages 12–24.
Understanding of trauma, community violence, and youth development principles; strong communication, documentation, and organizational skills.
Valid driver’s license and reliable transportation.
Availability to work flexible hours, including occasional evenings/weekends.
Desired Qualifications Prior experience with diversion programming or violence prevention initiatives.
Familiarity with juvenile court procedures, restorative justice models, and conflict resolution techniques.
Knowledge of community resources across the Eastern Shore and Prince George’s County.
Proficiency in using Azzly or similar electronic case management systems.
Bilingual in English/Spanish or other community-relevant languages is a plus.
