Forensic Case Manager
Elwyn invites candidates to join our multidisciplinary team as a Forensic Case Manager to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to ACCS clients under clinical supervision.
Ensures clients receive effective, understandable, and respectful care compatible with cultural beliefs and preferred language.
Along with the team, reviews required outcomes to evaluate community tenure and independence on an ongoing basis.
Provides service coordination (case management) for an assigned group of clients, including coordination and monitoring of activities, and implements, evaluates, and revises the overall Community Service Plan in collaboration with the client and team.
Practices a Housing First approach.
Utilizes evidence‑based assessment tools that support matching a client’s needs to services.
Develops the Community Service Plan in partnership with the client and team.
Provides de‑escalation techniques to manage crisis and maintain safety.
Participates in the Daily Organizational Meeting.
Documents client progress, activities, and outreaches in the Electronic Health Record.
Practices engagement skills across the adult lifespan.
Provides practical assistance, advocacy, coordination, problem‑solving, training, and supervision to help clients obtain daily living necessities and maintain safe housing.
Encourages participation in community activities and linkages supporting the client’s plan.
Performs other duties as assigned.
Pre‑Employment Education/Experience
Bachelor’s in behavioral science preferred or 5+ years of field experience considered.
3+ years of related experience working with individuals with mental health conditions preferred.
Preference for candidates who meet cultural and linguistic needs of the area.
Current valid driver’s license in state of residence with at least two years of U.S. driving experience and an acceptable driving record.
Relevant Post‑Job Offer Testing/Conditions
Knowledge and practice of FHR, DPH, and DMH policies, regulations, and standards.
Must complete ACCSModule Training within 30 days of hire.
Participate in other Elwyn required/recommended training per the training calendar.
Meet or exceed professional development goals.
Demonstrate fidelity to the ACCS model.
Establish relationships with internal and external supports for program functioning within the system.
Ongoing education and training in culturally and linguistically appropriate service delivery.
