As a Field Case Manager working with Medicaid members you’ll wear many hats and work in a variety of environments. You may interact with members leaving the hospital with new medications or diagnoses, perform home visits, and assist members with safe, effective transitions from care environments to where they live. You may act as an intermediary between providers and members as an educator, evaluator, service coordinator, community resource researcher and more. The result is fewer hospitalizations, ER visits and costly service gaps, and a less stressed, more effective health care system. You may have flexibility and autonomy to work from home and coordinate a visiting schedule that works for you and the members we serve.

If you are located in or within commutable driving distance to Olympia, Port Angeles, Tacoma, or Chehalis, Washington, you will have the flexibility to telecommute within the role’s requirements.
Primary Responsibilities Provide a complete continuum of quality care through close communication with members via phone or in-person interaction
Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels
Assist members with the transition from a care facility back to their home
May require some field work—meeting members in homes or facilities if telephonic engagement is difficult or at the request of management
Meet NCQA Population Health Management Standards
Meet state-specific contractual requirements related to the delivery of care management
Required Qualifications Current, unrestricted RN or behavioral health license in State of Washington
2+ years clinical experience in a hospital, acute care, home health/hospice, direct care or case management
1+ years of experience directly working with individuals with special healthcare needs
Computer/typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications
Access to high speed internet from home
Reliable transportation, a valid driver’s license, and the ability to travel up to 50% within a designated territory
Reside in or within commutable driving distance to Olympia, Port Angeles, Tacoma, or Chehalis, Washington
Preferred Qualifications Bachelor’s degree or greater
Certification in Case Management (CCM)
2+ years of clinical experience in a behavioral health setting
Case Management experience
Home care/field-based case management experience
Medicaid, Medicare, Managed Care experience
Experience with vulnerable subpopulations including children and youth with special health care needs, adults with serious mental illness, members with substance use disorders and members with other complex or multiple chronic conditions
Pay is based on several factors including local labor markets, education, work experience, and certifications. In addition to salary, benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (eligibility requirements apply). The hourly pay range for this role is $28.27 to $50.48 per hour for full-time employment. We comply with all applicable minimum wage laws.
