As part of our Optum At Home team, we assist patients in navigating the health care system and connecting them to support services, promoting wellness at home.
The DSNP program involves trained clinicians providing tailored interventions in collaboration with an interdisciplinary team, delivering care in patients’ homes and via telephonic support.
Schedule: Monday - Friday, 8am-5pm Central, with potential for 4x10 hour days after 6 months. No on-call or holiday work.
Territory: Approximately 60% fieldwork around Jonesboro, Fort Smith, or Springdale, AR, within a 60-mile radius, and remote work when not in the field.
Primary Responsibilities Coordinate Medicaid/Medicare benefits with a multidisciplinary team
Build community relationships and work with diverse populations, including adults with mental illness, substance use disorders, and chronic conditions
Assess members, establish goals, and develop care plans
Collaborate with team members and communicate with healthcare providers
Refer members to community resources and document in electronic health records
Educate members and families on health maintenance and follow-up
Participate in quality initiatives and team meetings
Maintain necessary credentials and demonstrate commitment to organizational values
Qualifications Master’s Degree in Social Work
LMSW, APSW, or LCSW in Arkansas
2+ years of social work experience
Experience with electronic medical records and MS Office
Ability to work in diverse settings with effective time management and communication skills
Valid driver’s license and reliable transportation; ability to travel within the territory
Preferred Qualifications Case Manager Certification (CCMC)
Experience with community resources, Medicaid, and duals populations
Field-based work experience
Physical & Mental Requirements Lift up to 10 pounds and push/pull up to 25 pounds
Sit and stand for extended periods
Operate office equipment and drive safely
Receive and follow instructions
