Description Gunnison County is seeking a FAST Case Manager to support the House Bill 14-51 Collaborative Management Project (Family Advocacy and Support Team – FAST) and the Statewide System of Care Implementation. The role ensures the mission and principles of FAST align with HB 1451, related MOU, and System of Care strategies, values, and principles. The Case Manager will collaborate with the school district to meet youth and family needs and participate in multidisciplinary teams (MDTs) such as FAST, Gunnison County Substance Abuse Prevention Program (GCSAPP), Early Childhood Council (ECC), and Child Welfare meetings as appropriate.
The Case Manager provides high-level student support intervention, coordinates services for youth and families experiencing legal, academic, personal, or medical/mental health difficulties, and helps remove barriers to success through goal-oriented, strength-based assessment to enhance holistic well-being. The role involves ISSTs and higher-level case management, facilitating team meetings, developing individualized case-management plans, coordinating implementation, and monitoring progress in partnership with involved agencies.
Note This position is currently supported by grant funding; employment beyond the current grant period is dependent on funding availability.

Pay & Hours The hourly range is from $29.51 to $33.59, depending on experience, plus partial benefits. The County pays monthly on the last weekday of the month.
Requirements Must possess a valid Colorado driver’s license.
Must complete ICS 100 and ICS 200 within the first year of employment.
Qualifications & Education Bachelor’s degree in a human services/social services field. Experience in clinical social work, counseling or case management support and participation in community collaborations is required. Bi-lingual (Spanish) is required.
