Job Summary
This is an ideal opportunity for an individual who enjoys working in a team atmosphere and is dedicated to providing both behavioral health care and social services to the individuals we serve in the community.
The FACT Program is a multi-disciplinary, clinical team that provides treatment, rehabilitation, and support services to adult persons with severe and persistent mental illness, promoting recovery, empowerment, self‑determination, and decision‑making. Responsibilities include advocacy, crisis intervention, assessments, evaluations, treatment planning, case management, medication management, community referrals, symptom education, vocational and educational services, and support for participants’ families and caregivers. The work is mostly community based, with occasional office time. Members use company vehicles or reimbursed personal vehicle mileage. The program operates 24/7 with rotating weekend and on‑call duties, and team members carry an emergency on‑call cell phone.
Responsibilities
Provides treatment, rehabilitation, and support services in accordance with the FACT model and PACT manual.
Ensures accuracy, completeness, and confidentiality of clinical records.
Conducts mental health status assessments.
Coordinates referrals to community providers and support services.
Assists with activities of daily living, transportation, communication, social and interpersonal relationship skills.
Provides education related to symptoms, medications, and mental health issues.
Serves as a primary case manager for an assigned group of individuals, promoting recovery, empowerment, self‑determination, and decision‑making for each FACT participant.
Qualifications
Bachelor’s degree in Human Services or a related field.
Previous experience working with adults with severe and persistent mental illness (schizophrenia, bipolar disorder, borderline personality disorder, etc.).
Valid Florida Driver’s License and insurance with coverage at least 50,000/100,000/50,000 split limits; no more than eight points on the license.
Ability to work independently, make sound decisions, and respond to emergencies.
Strong communication skills for interacting with patients, families, caregivers, community providers, and supervisory staff.
Experience working in home environments, closed offices, hospitals, nursing homes, or locked units.
Position Details
Full time, Monday through Friday (rotating weekends, on‑call, and holidays).
