About the Organization Mental Health Resource Center is a not-for-profit Florida corporation that provides a wide range of mental health and behavioral health care services to the community, including 24-hour emergency services, inpatient psychiatric services for children, adolescents, and adults, and outpatient services such as medication management, case management, and counseling.

The FACT Program is a multi-disciplinary, clinical team that assumes responsibility for directly providing needed treatment, rehabilitation and support services to adult persons with severe and persistent mental illness, promoting recovery, empowerment, self-determination, and decision-making for each FACT participant.
Most of a FACT team member's day is spent in the community completing duties described below. Only about 15% of time is spent in the office. Company vehicles are available; members may use their personal vehicles and are eligible for mileage reimbursement. The FACT team operates both during and after normal business hours, rotates holiday and weekend responsibilities, and team members rotate carrying an emergency on-call cell phone.
Responsibilities Provides treatment, rehabilitation, and support services in accordance with the FACT model, based on the Program for Assertive Community Treatment (PACT) manual
Ensures the accuracy, completeness, and confidentiality of clinical records
Conducts mental health status assessments
Coordinaties referrals to community providers and support services
Provides assistance with activities of daily living, transportation, communication, social and interpersonal relationship skills
Provides education related to symptoms, medications, and mental health issues
Serves as a primary case manager for an assigned group of individuals and promotes recovery, empowerment, self-determination, and decision-making for each FACT participant
Qualifications Bachelor’s degree in Human Services or a related field
Experience working with adults with severe and persistent mental illness (e.g., schizophrenia, bipolar disorder, borderline personality disorder)
Valid Florida Driver's License, automobile insurance of $200,000, and no more than eight points on license (per MHRC/RBHS vehicle driver requirements)
Ability to work with individuals in homes, closed offices, hospitals, nursing homes, and locked units
Ability to make sound independent decisions daily, including recognizing emergencies and life-threatening situations
Strong communication skills to interact with families, caregivers, community service providers, supervisory staff, and other department professionals
