Exceptional Needs Care Coordinator at Lifeways, Inc.
This role coordinates care for individuals with exceptional needs, managing services and fostering collaboration across care teams. Applicants will be responsible for processing care plans, coordinating placement, and ensuring compliance with regulations.
Essential Duties and Responsibilities
Process plan of care requests for service payments.
Coordinate with the team to gather necessary documents for foster home placement; track resident placement and progress; provide documentation to foster homes for Residential Care Plans.
Guardianship: Monthly billing processes for Long-Term Care Requests (application for admittance to the Oregon State Hospital); Interdisciplinary Team Meetings (IDT); system and process development.
Conduct face-to-face meetings with patients referred to Oregon State Hospital within 72 hours of referral to assess diversion from state hospital waitlist.
Maintain clear and professional email communications; respond promptly to calls and emails from internal staff and external agencies.
Multi-task, manage time, meet deadlines, and remain calm and professional under pressure.
Maintain confidentiality of Lifeways’ clients and records.
Perform all work duties in compliance with Lifeway’s policies/procedures, Oregon Administrative Rules, OSHA, and HIPAA regulations.
Required Qualifications
Bachelor’s degree and two years of experience, or an equivalent combination of education and experience.
Current driver’s license.
Successful completion of a background check.
Preferred Qualifications
Experience with individuals with severe or persistent mental illness, developmental disabilities, or substance use disorders.
Case Management experience.
