Hire Professionals has exciting opportunities for Case Managers for a growing client program in Pennsylvania.
As an Enrollment Case Manager, you will travel throughout the assigned county working in applicants’ places of residence, conducting intake visits for waiver applicants and acting for the eligibility determination.
Summary of Qualifications:
Traveling throughout assigned county conducting intake visits for waiver applicants and facilitating the eligibility determination process.
Providing assistance from the time individuals are identified as potential participants in a program to the time they are enrolled as participants.
Reviewing and screening the provider application and relevant documentation accompanying the application.
Initiating the development of individual service plans and obtaining department approval.
Assisting with developing relationships with community-based organizations, advocates, and stakeholders to gather feedback on improving the application process and removing barriers to enrolling in waiver programs.
Contacting providers to obtain missing or incomplete information from the provider application.
Working with Site Visit Field Staff to collect and verify results from site visits.
Education and Experience:
A minimum of one year of experience in public or private social work and a Bachelor’s Degree.
Two years of case work experience, including one year of experience performing assessments of clients’ functional ability to determine the need for institutional or community-based services.
