Responsibilities Serves as the lead in multidisciplinary planning and coordination of patient care across the healthcare continuum. Organizes the sequence of patient care to attain maximum levels of effectiveness and efficiency to reach the desired quality and outcomes. Utilizing a team approach, this position coordinates and organizes multiple strategies to ensure patients are managed with minimal overlapping roles and processes. Collaborates with physicians, nurses, other case managers, direct and indirect providers, community resources, and payers. Demonstrates Service Excellence at all times. Other duties as assigned.
