Job Description
Title: ECM Care Manager, Homeless Youth & High Utilizer Youth

Reports to: ECM Program Director

Location: Los Angeles County, CA (Hybrid)

Established in 1978, Affordable Living for the Aging (ALA) is a nonprofit organization that provides case management and affordable housing for high‑acuity populations in Los Angeles.

Program Overview
Enhanced Care Management (ECM) is a statewide benefit established by the Department of Health Care Services (DHCS) to provide a whole‑person approach to care for high‑need beneficiaries enrolled in Medi‑Cal Managed Care. The goal is to manage medical and psychosocial needs effectively, ensuring a well‑managed health condition and minimizing preventable hospital admissions and emergency department utilization.

Job Summary
The Care Manager is the primary point of contact for enrolled members, including those experiencing homelessness, mental illness, substance use disorders, or high emergency utilization. The Care Manager will:

conduct assessments and develop care plans for newly enrolled members;

schedule weekly or bi‑weekly meetings with members based on acuity;

participate in weekly case conferences with the Clinical Supervisor;

perform quarterly reassessments of all clients;

complete case notes in the case management program and provide quarterly metrics for external reporting;

initiate clinical consultations for difficult members and complex cases; and

coordinate care with providers and community resources.

Care Management Services (examples)

Coordinate access to health care, social services, and community resources at the member’s location.

Conduct comprehensive risk assessments and develop patient‑centered care plans.

Monitor implementation of care plans and recommend revisions as needed.

Liaise with primary care providers, specialists, behavioral health providers, and community resources.

Educate members on self‑management skills and recruit caregiver support.

Support health behavior change using motivational interviewing and trauma‑informed care practices.

Monitor treatment adherence.

Co‑participate in case conferences with providers.

Coordinate hospital discharge plans and transitional care.

Accompany members to medical visits and appointments as requested.

Qualifications

At least 3 years of care management or care coordination experience (e.g., LVN, Medical Assistant, Social Services).

Bachelor’s Degree in Social Work, Psychology, Public Health, or a related field.

Experience and interest working with homeless youth and high‑utilizer youth.

Understanding of evidence‑based practices, including motivational interviewing and trauma‑informed care.

Strong written and oral communication skills.

Proficiency in MS Word, Excel, and online case management software.

Reliable transportation, valid driver’s license, and proof of auto insurance meeting California minimum coverage.

Must complete and pass a background check and work verification.

Spanish‑speaking a plus.

Lived‑experience a plus.
