Position Summary
Under minimal supervision, work independently and together with other Enhanced Care Management (ECM) Case Managers and Hill Country Community Clinic teams. Along with other community agencies, provide outreach and direct case management services to help clients achieve wellness and recovery goals. ECM Case Managers provide engagement and support services, often to program participants engaging in behaviors that pose health and safety risks, by meeting individuals “where they are at” and providing non‑judgmental, non‑coercive services and resources. The ECM Case Manager is expected to become a leader in promoting a commitment to providing services using a harm reduction approach.
Essential Duties and Responsibilities
Initiate contact and establish positive rapport with clients/potential clients.
Ensure correct, concise, and consistent documentation in the Electronic Health Record (EHR).
Provide clients with empathy, encouragement, support, and facilitate problem solving.
Interview clients to identify areas of need, strengths, resources, and goals.
Conduct and record needs assessments and mental status examinations.
Administer PHQ‑9 assessments, measure blood pressure, and build care plans with the client leading care.
Help clients identify strengths and resources to address their needs/goals.
Provide crisis intervention and coordinate emergency care as needed.
Assist clients in obtaining basic needs (food, clothing, shelter, transportation).
Advocate for clients to obtain housing, public benefits, and other community resources.
Help clients develop budgeting and money management skills; coach activities of daily living, social skills, and communication.
Assist clients in finding and using opportunities and resources to improve quality of life (education, employment, volunteering, recreation).
Work creatively with team members to implement a “whatever it takes” approach to support client resilience, wellness, and recovery.
Complete daily documentation of interactions, assessments, interventions, and services provided.
Build collaborative relationships with clinic staff, social service providers, schools, employers, emergency personnel, and other community agencies.
Improve service quality by sharing knowledge of community resources, public benefit programs, work/volunteer opportunities.
Increase professional competence through available training opportunities.
Attend weekly team meetings and other meetings as appropriate.
Other projects and tasks as assigned.
Supervisory Responsibilities
This position has no direct supervisory responsibilities but collaborates with CARE Center staff and larger clinic staff as needed to serve clients and the community.

Qualifications

Education / Experience
B.S. degree preferred in social work or related study, or equivalent work experience.
Certificates and Licenses
CPR certificate preferred. Current and clean California driver’s license required.
Language Ability
Excellent communication skills. Ability to speak publicly and one‑on‑one persuasively; read, write, analyze, and interpret complex documents; respond effectively to sensitive inquiries or complaints.
Math Ability
Intermediate math skills sufficient for budgeting and personal finance management (addition, subtraction, multiplication, division).
Reasoning Ability
High reasoning skills. Ability to assess mental status, determine needs for emergency interventions, define problems, collect information, establish facts, and draw valid conclusions.
Computer Skills
Strong working knowledge of Windows operating systems, Microsoft Office (Word, Excel, Outlook), browsers, and related software.
Equipment Skills
Proficient with standard office equipment (computers, copiers, fax machines, printers, multi‑line telephone systems). Experienced driver comfortable transporting passengers.
Work Environment
Indoor business office, health clinic, transportation vehicle, agency office settings, schools, and patient homes. Moderate noise levels typical of office, computer, and printer use.
Physical Demands
Regularly sit, use hands, reach, stand, walk, write, and hear. Occasionally climb or balance, stoop, kneel, crouch, or crawl. Must lift up to 10 lbs regularly and up to 25 lbs occasionally. Requires good vision (near, distance, color, peripheral, depth).
