Overview
Job Description

Spectrum Healthcare Resources has an opportunity for a civilian Disease Management Registered Nurse at Whiteman Air Force Base, Missouri located just south of Knob Noster, MO.

Qualifications

Degree/Education: Graduate from an accredited college by the National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). An Associates Degree of Nursing; Bachelors of Science in Nursing is preferred. Additionally, current, full, active, and unrestricted license to practice as a Registered Nurse.

Experience: Must have at least 36 months of total nursing experience in a direct patient care clinical setting.

Certifications: BLS through American Heart Association.

Responsibilities

Identifies and stratifies individual patients and populations at risk for chronic diseases and co-morbid conditions using data from established data bases and facility treatment team referrals.

Facilitates identification, adoption, implementation and utilization of standardized clinical practice guidelines and protocols for management of specific diseases/conditions.

Identifies, develops, and executes disease management activities and interventions such as individual and/or group patient contacts and training classes.

Conducts thorough evaluation of the patient’s current physical, psychosocial and health status; documents treatment plans and matches level of care to clinical risk, readiness to change and health literacy.

Educates/trains team/facility staff on policies and procedures for utilization of guidelines, protocols and other disease management activities; fosters collaboration within the patient care team.

Identifies, collects, interprets and evaluates measurable outcomes of care within established time frames (quality of services, cost and cost savings, disease management and improvement).

Proactively coordinates with the Case Manager and Utilization Manager as needed.

Recognizes adverse signs and symptoms and responds quickly in emergency situations.

Completes in-person and telephonic evaluations to determine optimal time and location for patient management and follow-up care.

Prescribes and communicates treatment plans and patient teaching in accordance with established protocols; collects and assesses significant patient history information and provides patient teaching.

Operates and monitors specialized medical equipment (e.g., cardiac monitoring devices, EKG units, oxygen analyzers, nebulizers) as required.

Performs or assists with procedures such as medication administration, oxygen administration, bladder catheterizations, visual acuity tests, EKGs, eye/ear irrigation, tympanograms, splinting, phlebotomy, and other clinic-specific procedures per established protocols.

Performs nursing assessments, planning, implementation and evaluation of care; tele-health; medication administration; clinical nursing procedures and patient education.

Coordinates and participates in interdisciplinary team meetings, facility meetings, and Care Coordination meetings; shares knowledge gained from clinical practice and education relevant to nursing and disease management.
