Start Date: Monday, September 29, 2025

Training: 8am – 4:30pm CT. Due to the structured training schedule, new hires are unable to miss any days for the first 60 days of employment (9/29/2025 -11/29/2025). Unfortunately we are unable to make any exceptions.

Available Shifts:

Monday-Friday: 9am - 5:30pm CT and every 4th Saturday from 8am-4:30pmCT

Monday-Friday: 11am-7:30pm CT and every 4th Saturday from 8am-4:30pmCT

Monday-Friday:11:30am-8pm CT and every 4th Saturday from 8am-4:30pm CT

Responsibilities

Essential Job Functions:

Achieve clinical objectives by reviewing status of participants’ preventive health exams as defined by disease-specific Standards of Care guidelines to close gaps in care.

Contact participants by telephonic dialer system per Disease Management program design and call cadence for education, assessment, intervention, and behavioral goal setting.

Identify educational needs of participants and facilitate educational opportunities (education materials; refer to employer or health plan specific vendors or other Sharecare programs as appropriate).

Review participant’s functional status and support system to determine desired outcomes of care and needs for participant education.

Collaborate with the participant to develop a care plan addressing total healthcare needs.

Identify participant barriers to accessing health care services and advise on benefits and local resources to remove system barriers.

Make referrals to dietitian or coach colleagues to ensure continuity of care.

Accept warm transfer calls from non-RN colleagues for assessment and/or additional discussion with participant when necessary.

Advanced active listening skills.

Prior experience and proficiency with motivational interviewing and behavior change preferred.

Ability to thrive in a telephonic and omni-channel environment, while meeting quality and productivity metrics.

Self-directed, highly organized, and proactive.

Ability to identify and integrate quality improvement processes into practice.

Effective communication skills and ability to provide positive customer service to internal and external customers.

Proven success in influencing patient outcomes.

Qualifications
This position requires a responsive, committed individual who recognizes the impact of the health care delivery system on the clinical and financial outcomes of the member population and can identify and take action to facilitate system improvements.

Current Registered Nurse multi-state compact license in the state in which they reside and able to obtain additional state licensure; BSN preferred.

Minimum of 3 years recent experience in a clinical setting.

Basic knowledge of physiology and chronic disease appropriate for an RN.

Language Skills: Ability to communicate with members, other members of the team, physicians, and plan representatives. Effective oral and written communication skills. Ability to read, analyze, and interpret common scientific and technical journals.

Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret and organize data in an effective and useful manner; ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Computer Skills: Basic computer skills with the ability to learn and utilize new software; typing at least 40 WPM.

This position will be based in a home office which must satisfy all HIPAA requirements.
