Responsibilities
Responsible for the coordination of the various activities of the Case Management Department under the direction of the assigned Case Manager/Social Worker. Assist with development and implementation of discharge plans, case management referrals and authorizations, maintain and coordinate Medicaid state submission forms, interview patients/families and gather data to develop a safe patient‑focused Discharge Plan.
Qualifications
Required Qualifications
Knowledge of Discharge Planning/Utilization Management/Case Management terminology and functions, in both managed care and non‑managed care environments.
Experience in basic to intermediate computer skills/knowledge.
High School Diploma required.
Preferred Qualifications
Current BCLS certificate required upon hire and maintain current preferred.
Associate’s degree or LVN license preferred.
Experience in the hospital setting preferred.
Pay Transparency
Montclair Hospital Medical Center offers competitive compensation and a comprehensive benefits package. A reasonable compensation estimate for this role is $21.00 to $23.39 per hour. The exact starting compensation will be determined at the time of hiring.
