Detention Center Care Manager (Full Time, Cumberland County, North Carolina Based)

The Alliance Detention Center Care Manager (CM) is responsible for providing timely post‑release planning for detainees with complex needs due to Severe Mental Illness, Substance Use Disorders, Intellectual Disabilities, unmet Physical Health needs, and/or Social Determinants of Health. The CM engages detainees for up to 30 days post‑release to connect them to a Community Care Manager and needed resources such as medical providers, medication management, behavioral health providers, housing, insurance coverage, employment, transportation, food, and veteran services. The focus is to assist detainees with successful community re‑entry that prevents criminal justice recidivism and hospitalization.
This position is primarily remote; however, the successful candidate will be required to attend Alliance office business and team meetings as needed and will serve the Cumberland County area. The position is provisionally county funded and is potentially time‑limited unless further funding is secured.
Assessment & Care Planning
Meet with detainees and/or guardians within one business day of referral to establish rapport and complete a Care Needs Screening.
Make a best effort to meet same day or within one business day for detainees with a pattern of multiple short detainments to prevent rapid and repeated detainments.
Meet with detainees as needed for release planning; minimum one time weekly.
Partner with detainee and/or guardian to develop a post‑release plan based on the Care Needs Screening and adjust the plan as needed.
Apply a person‑centered approach that is sensitive to the detainee’s culture, values, and preferences.
Document assessment findings including formal and informal support systems, strengths, priorities/goals, SDOH, and care need gaps.
Develop a comprehensive, holistic care plan that integrates physical, behavioral, social, functional, and other identified needs.
Care Management and Collaboration
Accept referrals from detention center staff, whether Alliance members or non‑members.
Research treatment and diagnostic history of Alliance members and notify assigned Care Manager and enhanced providers of detainment.
Collaborate with Peer Support Specialists throughout engagement; coordinate efforts to achieve best possible outcomes.
Obtain Release of Information from detainees and/or guardians to collaborate with family and providers.
Assist detainees with developing healthy coping strategies, problem‑solving skills, and goal setting.
Provide education to detainees and/or guardians about available services and resources.
Complete referrals as needed for post‑release services.
Participate in interdisciplinary team meetings for resolution of complex needs.
Make every attempt to connect detainee with providers prior to release.
When possible, meet with detainee on release day, review the post‑release plan, and ensure they have needed contact information.
For detainees assigned to a Provider Led Entity, Standard Plan, or other health plan, provide linkage to Care Management and take lead if necessary.
Provide education to Alliance members about Tailored Plan, Medicaid Direct, and State‑Funded services, and assist with plan requests.
Engage with detainees in community up to 30 days post‑release to ensure connection with needed providers and resources; conduct warm hand‑off to assigned Community Care Manager.
Advocacy
Identify barriers to post‑release planning and raise awareness with supervisor and stakeholders.
Maintain confidentiality and privacy mandates during facilitation, collaboration, and coordination efforts.
Provide education and guidance on self‑determination.
Promote informed and shared decision making, autonomy, and self‑advocacy.
Connect detainees and/or informal supports with education to improve functioning, adaptive skills, and navigation of the service system.
Educate other healthcare and service providers on recognizing and respecting detainee strengths, needs, goals, and decisions.
Recognize and work to prevent disparities in accessing quality services and care.
Advocate for least restrictive services, timely transitions, and allocation of resources to optimize outcomes.
Recognize potential rising risk or escalation; consult supervisor and take lead when needed.
Professional Accountability
Act professionally in alignment with Alliance values.
Pursue professional knowledge and practice excellence to maintain competence in care management and human service delivery best practices.
Prioritize detainee safety; report safety concerns to relevant detention staff and consult supervisor.
Participate in data collection of outcomes, system needs, volume served, and position activities.
Document all contacts and release‑planning efforts in the Alliance electronic record as per guidelines.
Monitor detainee response to release planning during detainment and for 30 days post‑release.
Maintain compliance with local, state, and federal regulations and organization accreditation and certification standards.
Maintain professional license or certification as applicable.
Travel
Travel as needed to meet detainees, guardians, providers, and stakeholders.
Travel to Alliance offices and catchment area for meetings, trainings, and sponsored events.
Travel outside Alliance catchment areas as needed.
Minimum Requirements
Master’s degree in Human Services or related field from an accredited institution.
At least one year of full‑time post‑graduate experience providing care management, case management, care coordination, discharge planning, or utilization management to members with behavioral and physical health conditions in a behavioral health, medical, or managed care setting.
Must reside in North Carolina and have ability to travel as needed.
Preferred Credentials
Valid and active clinical license in North Carolina (LCSW, LMFT, LCAS, LCMHC, LPA, RN).
Certifications such as NACCM, NADD‑Specialist, Health Education Specialist, or CBIS.
Knowledge, Skills & Abilities
Knowledge of assessment and treatment of mental health, substance abuse, intelligential and developmental disabilities.
Knowledge of legal waivers, accreditation standards, and program practices/requirements.
Knowledge of Alliance Health benefit plans and network providers.
Ability to organize multiple tasks and manage caseload under pressure of deadlines.
Exceptional interpersonal and communication skills.
Extensive understanding of DSM (current version).
Knowledge of the MHSU/IDD service array and LME/MCO network.
Understanding of LME/MCO implementation of 1915(b/c) waivers and accreditation.
Skilled in assuring long‑ and short‑range goals and needs of individuals are addressed and updated.
Excellent computer skills in Microsoft Office.
Strong problem‑solving, negotiation, arbitration, and conflict resolution skills.
Detail‑oriented; able to organize multiple tasks and priorities.
Ability to make independent decisions based on relevant facts and maintain effective relationships.
Ability to adapt focus to meet changing priorities.
High level of diplomacy and discretion required for negotiating and resolving issues with minimal assistance.
Salary & Benefits
$28.96 – $34.62 per hour. Exact compensation will be determined based on candidate’s education, experience, market data, and internal equity.
