Counselor II/Case Manager - A Way Out

The Counselor II/Case Manager will be responsible for assisting A Way Out (AWO) program clients on behalf of the LCHD/CHC and Lake County partners. Provides linkage/case management and support to AWO participants while in treatment and post-treatment. Increases awareness of treatment options and resources. Works with the research team to track participation and grant-related outcomes.
Scheduled Hours: 37.5 hours a week, Monday - Friday (8:30am-4:30pm)
Responsibilities include:
Facilitates with AWO participants and collaborates with the treatment team to develop and review treatment goals and referrals both internally and externally.
Educates community entities and individuals on AWO program and substance abuse issues.
Provides an atmosphere of customer service in assuring that participants are treated in a respectful and friendly manner.
Works with the evaluation partner to compile and report data.
Supports individuals participating in the AWO program, including managing financial resources available for participant support.
Assists clients in transitioning from inpatient to community services. Helps secure and maintain entitlements.
Organizes and participates in treatment-related clinical conferences and staffings.
As needed, assigned on-call during regular working hours. As part of that function, employees will work directly with AWO participants post-screening to connect them to the best available resources.
Perform community outreach to AWO participants to enhance engagement with and follow through on treatment goals.
Provides education and direction to clients, family members and/or significant others.
Qualifications:
Bachelor’s degree in a related field, or
Bachelor’s in any field and two (2) years’ experience, or
High School diploma or GED and five (5) years’ experience
Requires use of personal vehicle when an LCHD fleet vehicle is not available for case management activities, client appointments, crisis response, community outreach, or other job-related activities.
CERTIFICATION: Preferably certified as an Alcohol/Drug Abuse Counselor by the Illinois Alcohol and Other Drug Abuse Professional Certification Board.
