The Connections Care Coordinator is responsible for maintaining and monitoring the memory care (Connections) holistic wellness program, including but not restricted to: assisting the Director of Engagement with input into resident assessments, leading neuroplasticity programming (Connect‑Move‑Learn cycles), muscular strength, aerobic and flexibility training, wellness promotion, social‑recreational activities and outings and volunteer program development and community resource coordination.

Highlights Of The Connections Care Coordinator Position

High school diploma or equivalent

Minimum 1 year experience activities or recreation service experience

Certified Dementia Practitioner or eligible for certification.

Willingness to work in a hands‑on role with residents and staff.

Must be able to drive a company vehicle (town car and 14 passenger van)

Must possess a current driver's license and ability to meet approved driver requirements
