Co-Responder Case Manager – Part Time | Montrose, CO

The Co-Responder Case Manager works in collaboration with law enforcement or emergency medical technicians to respond to community members whose need for urgent intervention was reported through 911. The Co-Responder team specializes in de-escalation, problem-solving, risk assessment, and community resource linkage. The Co-Responder works closely with the Montrose Police Department, Montrose County Sheriff’s Office, and partner agencies in Delta County to improve interactions between law enforcement, first responders, and the people they serve. Communities recognize savings by connecting people to appropriate services and resources rather than higher-level interventions.

The Case Manager uses crisis intervention approaches to provide intervention, support, risk evaluation, and triage services for populations utilizing the 911 system within Montrose and Delta Counties. The Case Manager serves as a first responder to connect individuals experiencing mental health, substance use, and social determinants of health to resources. The role includes patient continuity of care, brief counseling, referral, and prompt documentation, with a strong focus on community resourcing and linkage.

Responsibilities

Respond with crisis intervention and connect individuals to appropriate services.

Collaborate with law enforcement and first responders; participate in de-escalation and risk assessment.

Provide brief counseling, referrals, and timely documentation.

Engage in community resource linkage and continuity of care across Montrose and Delta Counties.

Deliver services in various community locations and coordinate with partner agencies.

Qualifications

Bachelor’s degree (Master’s degree preferred) in a recognized mental health profession.

Experience in an emergency service setting or demonstrated competency with de-escalation in a high-stress environment; crisis intervention experience preferred.

Driver’s License: current license, ability to be commercially insured, and insurable driving record.

Ability to complete training with law enforcement including basic tactical intervention, scene safety, basic firearm safety, defensive driving, CAD, and radio systems.

Required Skills

HIPAA knowledge and compliance; strong organizational and follow-through abilities.

Excellent interpersonal and communication skills; self-directed work style.

Knowledge of crisis intervention approaches and applications to clients with emotional, behavioral, and substance use problems.

Familiarity with Brief Solution Focused Therapy, Motivational Interviewing, and DBT coaching skills.

Ability to perform mental status evaluations, risk assessment, and service coordination.

Understanding of involuntary mental health holds (27-65), EC/IC processes; HIPAA and 42 CFR Pt. 2.

Knowledge of community health and behavioral health resources and eligibility criteria.

Proficiency with Windows applications (Word, Excel, Outlook) and basic office equipment.

Ability to organize and prioritize workload in a dynamic environment; follow universal precautions.

Travel required.

Salary
Co-Responder Case Manager: Base Rate $20.50 ($19.50 + Locale $1.00) – up to $23.34/hour, non-exempt.

Physical Demands
This position operates in community settings with travel in a vehicle; sitting for long periods, potential exertion during response, and occasional lifting up to 25 pounds. May be exposed to stressful situations and potential exposure to communicable disease. Reasonable accommodations may be made to enable performance of essential functions.

Eligibility & Contact

Annual Flu immunization and Annual TB screening.

CPI training within 90 days of hire; BLS PRO certification within 90 days of hire.

Current driver’s license with insurability for Axis Health System insurance.
