Summary: Under general direction, collects and analyzes data, identifies expected outcomes for each patient, develops a plan of care with interventions, and continually evaluates progress toward outcomes. Works collaboratively with the specialty healthcare team and other disciplines.

Responsibilities

Use critical thinking and problem-solving to coordinate health care services for a defined population.

Apply advanced nursing specialty knowledge to assess, plan, implement, evaluate, and interact with patients.

Evaluate options and services to meet individual health needs; develop and maintain patient tracking and reporting programs to identify gaps in care.

Coordinate care to support ANTHC’s mission and vision; participate in unit-based councils and process improvement activities.

Independently collect patient health data, analyze it to determine diagnoses, identify outcomes, develop and implement the plan of care, and evaluate progress with the care team.

Actively evaluate the quality and effectiveness of nursing practice; pursue quality of care initiatives and professional development.

Adhere to Standards of Practice for Case Management and the ANA Code of Ethics; contribute to shared governance and act as a consultant on specialty issues.

Deliver care ethically and nonjudgmentally, respecting patient autonomy, dignity, and rights; collaborate with patients, families, and other providers on goals and plans of care.

Consider safety, evidence-based practice, and cost when planning and delivering care; assist patients and families in identifying and securing appropriate services, including coordination with payers.

Plan, coordinate, and collaborate with external facilities for complex subspecialty needs not provided at ANMC; triage patients via phone, email, and walk-in situations.

Provide extensive patient education on complex specialty issues and disease processes; deliver timely nursing care per established standards.

Conduct program-specific nursing assessments, interventions, and evaluations; share specialty information with colleagues to improve overall nursing care.

Perform other duties as assigned.

Qualifications

Minimum Education: Associate degree in Nursing.

Minimum Experience: Non-supervisory - Five (5) years of professional work experience in health education, health-related research, and/or health-related program development, implementation, and evaluation. An equivalent combination of relevant education and/or training may be substituted for experience.

Minimum Certification: Current registered nurse license in the State of Alaska; current Basic Life Support (BLS) required.

Preferred Education: Bachelor’s Degree in Nursing.

Preferred Experience: Experience in the Alaska Tribal Health System.

Preferred Certification: National certification in Case Management through the ANCC or other specialty certifications.

Knowledge, Skills, and Abilities

Knowledge of nursing science, principles, techniques, and procedures for patient care.

Knowledge of healthcare terminology, anatomy, physiology, and disease concepts.

Ability to apply evidence-based practice to develop plans of care and interventions.

Knowledge of the hospital environment and how services interact; knowledge of Alaskan culture.

Ability to provide theory-based care and work within a shared governance environment; strong critical thinking skills.

Strong communication and collaboration skills to coordinate with patients, families, and teams; ability to triage and manage complex cases.

Proficiency with computer software and data management relevant to nursing practice.
