Responsibilities

Assist in developing and coordinating holistic care management activities for long-term care members

Evaluate member needs and recommend or facilitate plans for optimal healthcare outcomes

Coordinate services between members, families, and care provider teams to ensure accessibility

Monitor and document care plans and member status to ensure compliance with regulations

Provide education to members and their families on available services and healthcare benefits

Qualifications

Bachelor’s degree and 1 year of related experience, or equivalent experience

For specific plans, candidates must meet designated educational and licensure criteria (e.g., RN licensed in Illinois, social work degree)

Experience working with diverse populations, including those with disabilities and substance abuse issues

Ability to evaluate resources and recommend plans for member care

Strong communication skills to interact with members, families, and healthcare providers

Preferred Qualifications

Experience in case management or working with long-term care services

Familiarity with community resources and support services for vulnerable populations

Knowledge of healthcare regulations and compliance standards

Additional certifications in human services or related fields

Experience working with diverse family systems and cultural backgrounds
