Overview Clinical Case Manager - OC Interim Housing (JR 5280) at PATH — Santa Ana, CA 92704. Salary: $33.09 to $40.19 per hour. Full Time, Non-Exempt. Driving required. Location-based work; PATH does not offer fully remote positions.
PATH is seeking master’s level social services professionals to join our Orange County Interim Housing team as a Clinical Case Manager. This position provides therapeutic and case management services to support the guests residing at PATH's Yale Navigation Center in Santa Ana, with the goal of improving wellness and housing stability.
Responsibilities Facilitate psychosocial rehabilitation groups for guests as required.
Accept and process referrals from funders.
Conduct intakes and needs assessments; collaborate with guests to develop Individualized Service Plans (ISP) addressing barriers to services, housing, and health care, as well as short- and long-term goals.
Meet with guests to support progression toward ISP goals.
Provide ongoing case management support for ISP goals, including assistance with benefits establishment, obtaining identification documents, employment linkage, housing referrals, and accessing health care.
Utilize and develop community resources to broker and link guests to services.
Maintain documentation as required by the program contract and PATH policies.
What You Bring Maintain and safeguard confidential information according to HIPAA standards.
HMIS experience (preferred).
Knowledge or experience with case management techniques (e.g., Critical Time Intervention, harm reduction, crisis intervention, motivational interviewing).
High tolerance and understanding for individuals with urgent and multiple needs.
Ability to work independently and as part of a team; mature judgment; self-motivated and proactive.
Strong written and verbal communication skills; ability to prioritize and coordinate multiple demands in a high-pressure environment.
Qualifications Preferred Qualifications Current and active licensure or registration with the California Board of Behavioral Sciences (e.g., ACSW, LCSW).
Minimum Qualifications Master’s Degree in Social Work or equivalent behavioral health degree eligible for licensure through the CA Board of Behavioral Sciences (e.g., MSW, MFT).
One year of experience in healthcare, case management, homeless services, or working with vulnerable populations.
Mandatory Requirements Be able and willing to work flexible hours, including evenings or weekends.
Employment eligibility verification.
CPR/First Aid training (or willingness to obtain).
Complete the following as a condition of hire: Tuberculosis Test, Background Screening, Drug Test.
Driving Driving is an essential function of this position.
Have reliable transportation; valid driver’s license; proof of insurance and ownership for personal vehicles used during work duties; ability to qualify for PATH’s insurance coverage.
