Founded in 1971, BHI provides social rehabilitation including a full range of Medi-Cal services to adults with co-occurring serious mental health and substance use disorders.

Location: Oakland, CA 94162. Office Hours: Monday-Friday 8:30 am–5:00 pm.

Position Summary
Clinical Case Manager – 50% of time. Under the general direction of the Program Director, the Part-Time Clinical Case Manager is responsible for providing individual case management and rehabilitative individual and group therapies as part of an interdisciplinary team. Bonita House Inc.’s Supported Independent Living (SIL) Service Team provides intensive outpatient mental health and case management services to individuals with co-occurring serious mental illness and substance use disorders who live in Alameda County.

Responsibilities — Clinical Case Manager

Provide intensive outpatient case management, rehabilitative therapy and support to adults with mental health and/or substance use symptoms.

Help organize and lead social and recreational activities to help clients structure their time, increase social experience, and provide opportunities to practice social skills.

Must complete individual therapy, biopsychosocial assessments including ANSA's and sound clinical documentation that supports client treatment plans.

Actively participate as a team member, and present updates at each weekly team meeting.

Develop a keen understanding of Welligent and billing codes in order to document and bill Medi-Cal; appropriately and effectively assist in training new staff.

Assist staff with completing standard documentation requirements as needed.

Oversee therapeutic milieu including greeting members, ensuring they sign in/out, engaging with members, coordinating daily room use schedule, and maintaining a safe and welcoming environment.

Participate in clinical supervision with the agency QI Clinical Supervisor to ensure appropriate treatment and sign off on assessments; gain hours towards licensure if applicable.

Assist at-risk Medi-Cal recipients or potential eligibles to understand the need for mental health services.

Encourage reluctant and difficult Medi-Cal recipients/potential eligibles to accept needed mental health and healthcare services.

Operate a motor vehicle to include safe transport of clients to various appointments across Alameda County.

Coordinate with staff to gather information about an individual’s health and mental health; maintain documentation accordingly.

Document client progress to maintain a permanent record of client activity according to established methods and procedures, meeting agency and County documentation requirements.

Help clients and family members identify, understand, and combat stigma and discrimination associated with mental illness; develop strategies to reduce self-stigma.

Collaborate with the team to protect clients’ rights and help clients understand rights and grievance procedures; support clients with filing, mediating, and resolving complaints.

Provide ongoing assessment, problem-solving, side-by-side services, skill teaching, support (prompts, assignments, encouragement), and environmental adaptations to assist clients with challenges in employment, housing, and activities of daily living.

Complete timely clinical documentation.

Participate in staff meetings.

Perform other duties as needed.

Qualifications And Requirements — Clinical Case Manager

Master's degree in psychology, Social Work, Counseling, or a related field; active registration with BBS required.

BHI conducts background checks on all candidates who have received a conditional job offer. Satisfactory completion of background checks is a requirement for employment with the agency.

A valid driver’s license, reliable personal vehicle, personal auto insurance as required by law, and an MVR sufficient to obtain and maintain insurability under agency auto liability policies are essential job requirements.

Local travel as required to fulfill primary job responsibilities.

Knowledge Skills & Abilities — Clinical Case Manager

Must demonstrate an ability to work independently and as a member of the team.

Knowledge of mental health and substance use symptomology, treatment modalities and Community resources.

Ability to meet clients where they are, establish positive rapport and provide therapeutic assessment, problem solving, care planning and goal setting with individuals.

65% or 13 hours per week of total scheduled work hours (20) spent in the community engaging clients, providing case management services, and providing follow-along.

Demonstrated ability to effectively and sensitively provide care to people from different cultural backgrounds.

Passion and understanding of how to provide low-barrier outreach.

Understanding and ability to implement and support social concepts including Harm Reduction, Stages of Change, Trauma-Informed Care, and Motivational Interviewing.

Understanding and practice of culturally sensitive direct service.

Ability to effectively intervene in crisis situations using de-escalation techniques.

Excellent verbal and written communication skills and the ability to present information clearly.

Excellent interpersonal, negotiation, and conflict resolution skills.

Employment Specialist — 50% of time
Under the general direction of the Program Director, the Employment Specialist is responsible to actively outreach, engage, and provide a full range of services to 10-15 adults in Alameda County who have mental health and substance abuse challenges as part of an interdisciplinary team. Bonita House Inc.’s SIL Service Team provides intensive outpatient mental health and case management services to individuals with co-occurring serious mental illness and substance use disorders who live in Alameda County.

Qualifications — Employment Specialist

High School Diploma or Associate degree with 4 years of employment services experience.

Demonstrated ability to work as an effective team.

Knowledge of various job development strategies, local labor market trends in a range of occupational fields, and community resources.

Excellent computer skills including familiarity with Microsoft Office and ability to learn EHR systems.

Understand and follow HIPAA and PHI guidelines.

Ability to maintain confidentiality, professional demeanor and positively represent the organization at all times.

Oral and written communication—speaks clearly and persuasively; completes written progress notes and other communications.

Demonstrated cultural sensitivity.

Must have a valid California Driver License and a vehicle; proof of personal vehicle insurance may be required.

Must be able to meet the Immigration Reform Act of 1986.

Prior sales, marketing, or coaching experience

Bilingual in English/Spanish or other languages

Assists clients in obtaining information about benefits (e.g., SSI, Medicare) and impact of employment on benefits.

Comply with DOR regulations, CARF standards, and agency policies and procedures.

Assist clients in exploring educational and vocational training opportunities with a focus on job readiness.

Conducts job site analyses and makes decisions regarding employer suitability; maintains knowledge of ADA; educates employers on ADA and benefits of employing people with disabilities; identify barriers and provide solutions.

Provides education and support to employers including negotiating accommodations and follow-up with employer.

Provides outreach services as necessary to clients who disengage; uses multiple outreach methods.

Physical Requirements
Physical demands include continual sitting, typing, and repetitive computer use for up to 8 hours a day. The position also requires frequent communication with others, including hearing and speaking clearly with individuals and groups, in person and via phone and video calls. Ability to lift 15 pounds at times. Regular local travel and operation of a motor vehicle is required.
