Summary Women's Substance Abuse Services Clinical Case Manager will provide individual and group counseling to residents. The Clinical Case Manager will carry a caseload of up to eight clients. These clients will be primarily dual diagnosed or high-risk individuals.
Essential Duties & Responsibilities Complete Clinical Intake within the first week of admission
Complete Treatment Plans within the first 2 weeks of admission and update as needed
Be proficient in the use of the electronic Medical Record for each case
Meet weekly with assigned individuals
Participate in Clinical Supervision
Input entries into the Medical Record in line with the service line procedure
Participate in the on-call Rotation
Expand knowledge and participate in trainings
Understand DPH regulations and CARF standards as applicable
Provide transportation as needed
Establish contacts with DCF, Guardian, doctors and therapists to involve all parties in the treatment process
Make outside referrals as needed
Attend weekly staff meetings
Provide weekend, holiday and emergency coverage
Other duties as assigned
Required Education / Experience Bachelor's degree preferred or clinical licensure preferred
A minimum of five years' experience in the field
Individual and group counseling experience
Must have MA driver's license
Required Skills / Knowledge Strong written and verbal skills
Ability to maintain records accurately and efficiently
Capable of responding to crisis situations
If recovering, two years continuous sobriety required
Ability to work independently as well as part of a team
