Job Summary
Provides oversight and coordination of care for members
Coordination of care across the continuum of care while adhering to professional standards
Perform case management and practice and meeting regulatory standards
Minimum Qualifications
Bachelor's degree (BA/BS) from four-year college or university; or one to two years' related experience and/or training; or equivalent combination of education and experience
Licensed Medical Professional required
Certified Case Manager credential is required within 12 months of hire or appointment
Must be able to travel between locations and perform occasional after hours, weekend and holiday work
