Overview We are seeking a compassionate, skilled, and self-motivated Registered Nurse (RN) to join our Home Health team as a Clinical Case Manager. In this role, you will provide direct nursing care to patients in their homes, following an individualized plan of care under the direction of a physician. The Home Health RN is responsible for assessing patient needs, developing care plans, and delivering high-quality, patient-centered care while promoting recovery, independence, and overall health.
Responsibilities Conduct initial and ongoing assessments of patient health status and needs.
Develop, implement, and revise individualized care plans in collaboration with physicians and other healthcare team members.
Administer medications and treatments as prescribed by the physician.
Monitor and document patient responses to treatment and interventions.
Educate patients and family members on disease processes, self-care techniques, and health maintenance.
Coordinate care with other health professionals, including physical therapists, social workers, and home health aides.
Ensure compliance with all clinical and regulatory standards including HIPAA, OSHA, and CMS guidelines.
Maintain accurate, timely, and complete documentation in the Electronic Medical Record (EMR) system.
Participate in on-call rotations, team meetings, and case conferences as required.
Qualifications Current RN license in the state of practice (required).
Minimum of 1 year of nursing experience, preferably in home health, hospice, or acute care.
BLS Certification (Basic Life Support) – current and valid.
Valid driver’s license, reliable transportation, and proof of auto insurance.
Excellent clinical, communication, and time-management skills.
Ability to work independently and manage multiple patient visits per day.
Proficiency with electronic charting systems (EMR).
Experience with OASIS documentation and Medicare-certified home health agencies.
Physical Demands Ability to travel to patient homes and work in various home environments.
May require lifting, transferring, and positioning patients.
Frequent standing, walking, bending, and reaching.
