Job Description Summary
The Clinical Transition Specialist is an experienced professional who uses sales techniques to consult discharging patients, their families and facility staff about how Option Care’s products and services will be facilitated at an alternate site. The role ensures proper placement of patients within the Home Health Care setting by assessing patients, collecting pre‑admission information, and collaborating with internal intake and external case managers or discharge planners to implement an effective treatment plan. Additionally, the specialist builds relationships with referral sources and partners with Account Managers to grow referral rates and achieve sales goals.

Job Responsibilities

Proactively maintain and grow relationships with referral sources to increase sales and patient starts, serving as the key point of contact representing Option Care.

Interact with area service providers to sell Option Care services and assess patient eligibility for alternate site care.

Use sales techniques to educate patients, families and facility staff about Option Care’s services and products.

Provide hands‑on, in‑person education to patients and caregivers to facilitate timely discharge and therapy independence.

Effectively communicate with agency staff, medical teams, patients and families throughout the discharge process to implement an effective treatment plan.

Collect, review and complete pre‑admission information and secure related sign‑offs.

Partner with Account Managers and Regional Sales Directors to create and execute area business growth plans.

Communicate frequently with Account Managers to discuss opportunities, assess progress, and provide feedback related to promoting Option Care services.

Collaborate with marketing staff to deliver educational and promotional programs to patients, caregivers and referral sources.

Review patient medical records to obtain relevant history and insurance payor information, and communicate findings to patient registration.

Initiate care transition coordination with referral sources and internal partners to ensure seamless transitions to home or ATS, and participate in data collection for therapy start and patient tracking.

Maintain confidentiality of patient and proprietary information, observing legal and ethical guidelines.

Provide oversight and input to providers regarding patient assessment, treatment process, and transition to home care.

Coordinate communication with other providers and arrange for special medical supplies or appliances as needed.

Basic Education and Experience Requirements

Licensed Registered Nurse (RN) (or licensed pharmacist) in the state of practice.

At least 2 years of experience in the healthcare industry.

Basic Qualifications

Experience establishing and maintaining relationships with individuals and vendors at all organization levels.

Knowledge of standard practices and current relevant standards (e.g., ACHC, URAC).

Strong customer service skills, meeting quality standards and evaluating satisfaction.

Proficiency with basic PC applications: Internet, email, Microsoft Word, Excel, PowerPoint.

Credential for reliable transportation with a valid driver’s license and proof of insurance.

Ability to plan, organize and deliver presentations.

Travel Requirements
Travel to and from partnership facilities, community hospitals, and medical practice offices to process referrals and provide education and training support to patients, caregivers, and referral sources.

Preferred Qualifications

Hands‑on experience with home or alternate site infusion or discharge planning.

Experience growing service provider partnerships.
