Job Description Summary
Responsible for the transition of discharging patients by educating patients, families, and facility staff about Option Care services and products; assessing patient suitability for home health care; collecting preadmission information; and building strong relationships with referral sources to achieve sales goals.
Job Responsibilities
Proactively maintain and grow relationships with referral sources to increase sales and patient starts.
Serve as the key point of contact and representative of Option Care, providing education, assistance, and service to referral sources.
Interact with area service providers daily to sell Option Care services to discharge patients, conducting patient assessments to ensure a viable candidate for alternative site care.
Use sales techniques to educate patients, families, and facility staff about Option Care services and products.
Provide hands‑on, in‑person education to patients and caregivers to facilitate timely discharge and therapy independence.
Effectively communicate with agency staff, medical teams, patients, and families throughout the discharge process to implement an effective treatment plan.
Collect, review, and complete pre‑admission information and secure related sign‑off.
Partner with Account Managers and Regional Sales Directors to create and execute area business and growth plans.
Communicate frequently with Account Managers to discuss opportunities, assess progress, and provide feedback on promoting Option Care services.
Deliver educational and promotional programs to patients/caregivers and referral sources in partnership with marketing staff.
Review patient medical records to obtain pertinent history and insurance payor information and communicate this to registration.
Initiate care transition coordination with referral sources and internal partners to ensure seamless patient transitions to home or ATS, facilitating order transfers via phone, fax, or e‑prescribing.
Maintain confidentiality of patient and proprietary information, observing legal guidelines.
Provide oversight and input to providers regarding patient assessment, treatment, and transition to home care.
Serve as a point of contact, coordination, and communication with other providers.
Arrange availability of any special medical supplies or appliances.
Basic Education and Experience Requirements
Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice with at least 2 years of experience in the healthcare industry.
Basic Qualifications
Experience establishing and maintaining relationships with individuals at all levels of the organization in the business community and with vendors.
Experience applying knowledge of standard practices for all services offered and current relevant standards (e.g., ACHC, URAC).
Experience providing customer service to internal and external customers, meeting quality service standards and evaluating customer satisfaction.
Basic PC skills: competent use of internet, email, Microsoft Word, Excel, and PowerPoint.
Experience identifying operational issues and recommending and implementing strategies to resolve and improve processes.
Reliable transportation to travel to care facilities, home visits, and multiple hospitals; requires a valid driver’s license and proof of insurance if a personal vehicle is used.
Ability to plan, organize, and make presentations.
Travel Requirements
Travel to and from partner facilities, community hospitals, and medical practice offices to sell Option Care services, process referrals, and provide live education and training support to patients/caregivers and referral sources.
Preferred Qualifications
Hands‑on home or alternate site infusion or discharge planning experience.
Experience growing service provider partnerships.
