Clinical Care Transition Educator / Registered Nurse

Opportunity Overview
Option Care Health, the largest independent home and alternate-site infusion services provider in the United States, is expanding its clinical team. We are looking for a dedicated RN to educate patients, caregivers, and facility staff on our care delivery model.
Job Responsibilities
Evaluate, educate, and train patients, caregivers, and facility staff on how OCH services & products will be facilitated in alternative site, in-home, or virtual settings to ensure successful transition.
Conduct patient assessments to determine viability for the OCH care delivery model.
Partner with the OCH sales team to understand educational needs and develop improvements to increase patient transition volume.
Maintain confidentiality of patient and proprietary information in accordance with legal and company policies.
Provide proper documentation of education using OCH applications and technology.
Coordinate the transition of patients from hospital to home or alternate care settings, collaborating with facility staff, providers, and internal teams to ensure timely discharge, therapy education, and continuity of care.
Provide in‑person patient and caregiver education, gather pre‑admission and insurance information, and facilitate initiation of services to support safe and effective care at home.
Supervisory Responsibilities
No supervisory responsibilities (i.e., no hiring, performance reviews, or pay decisions).
Basic Education and Experience Requirements
Active and unrestricted Registered Nurse (RN) license.
Minimum of 2 years of experience in the healthcare industry.
Basic Qualifications
Establish and maintain relationships with individuals at all levels of the organization, the business community, and with vendors.
Apply knowledge of standard practices for all services offered, including current relevant standards (ACHC, URAC).
Provide customer service to internal and external customers, meeting quality standards and evaluating customer satisfaction.
Basic PC skills: Internet, email, Microsoft Word, Excel, PowerPoint.
Identify operational issues and recommend and implement strategies to resolve and improve processes.
Reliable transportation and driver’s license to travel to care facilities, home visits, and hospitals.
Willingness to obtain nursing licensure in additional states if needed.
Can plan, organize, and make presentations.
Preferred Qualifications & Interests
Hands‑on home or alternate‑site infusion or discharge planning experience.
Experience growing service provider partnerships.
Travel Requirements
100% local travel to and from partner facilities, community hospitals, and practice offices.
Salary
$38.84 – $64.72 per hour, based on education, experience, knowledge, skills, and abilities.
