Job Summary
The Care Transition Coordinator is an experienced sales professional who uses sales techniques to sell Option Care products and services to patients discharging from hospital care. They educate patients, families, and facility staff about how services and products will be facilitated at an alternative site and ensure proper placement of patients within the Home Health Care setting through collaboration with internal and external partners.
Key Responsibilities
Proactively maintain and grow relationships with referral sources to increase sales and patient starts.
Serve as the key point of contact and representative of Option Care to provide education, assistance, and service to referral sources.
Interact with area service providers daily to sell Option Care services that assist in the care of discharging patients.
Work with the clinical team to determine if a patient is a viable candidate for alternative site care.
Use sales techniques to educate patients, families, and facility staff about the services and products provided by Option Care.
Effectively communicate with agency staff, medical team, patients, and family throughout the discharge process to implement an effective treatment plan.
Collect, review, and complete preadmission information and secure related sign‑off.
Partner with Account Manager and Regional Sales Director to create and execute area business and growth plans.
Communicate frequently with Account Manager to discuss opportunities, assess progress, and provide feedback related to promoting the services of Option Care.
Coordinate educational and promotional programs with marketing staff for patients/caregivers and referral sources.
Initiate care transition coordination with referral sources and internal partners to ensure seamless patient transitions to home or ATS.
Facilitate the transfer of orders via phone, fax, and e‑prescribing as part of data collection for therapy start and patient tracking.
Review the patient’s medical record for pertinent medical history and insurance payor information and communicate this to the registration department.
Maintain confidentiality of patient and proprietary information following applicable legal guidelines.
Provide oversight and input to providers regarding patient transition to home care.
Serve as a point of contact, coordination, and communication with other providers.
Arrange for special medical supplies or appliances to be available.
Education and Experience Requirements
High School Diploma with 2 years of experience in applicable home infusion or post‑acute infusion services, OR
1 year of similar experience with an associate’s degree or LPN.
Basic Qualifications
Experience establishing and maintaining relationships with individuals at all levels of the business community and with vendors.
Experience applying knowledge of standard practices for all services offered, including relevant standards such as URAC and ACHC.
Experience providing customer service to internal and external customers, meeting quality standards, and evaluating customer satisfaction.
Basic PC skills: internet, email, Microsoft Word, Excel, PowerPoint.
Experience identifying operational issues and recommending and implementing improvement strategies.
Access to reliable transportation; if using a personal vehicle, a valid driver’s license and proof of insurance required.
Ability to plan, organize, and make presentations.
Travel Requirements
Travel to and from partnership facilities, community hospitals, and medical practice offices to sell services, process referrals, and provide education and training support to patient/caregiver and referral sources.
Preferred Qualifications
Experience growing service provider partnerships.
Compensation
Pay Range: $75,412.83 – $113,452.41, depending on education, experience, skills, and internal equity.
