The Clinical Care Coordinator is responsible for coordinating multiple aspects of care for patients with preventative care needs, chronic diseases, and care opportunities within the medical group. Daily work includes ordering laboratory tests or imaging exams per protocol to facilitate patient screenings, management of preventative care, and supporting optimization of chronic disease treatments as prioritized.

Required Qualifications 2 years – Work experience in a healthcare setting.
Experience with electronic medical records and/or computerized billing systems.
Epic or other Electronic Health Record /care management software experience.
