The mission of the Coordinator Clinical Care is to identify high risk patients and help coordinate resources to facilitate care, improve health outcomes, and reduce avoidable hospitalizations and re-admissions. Required Qualifications
Graduate of school for medical assistants and certificate.
Coursework/Training: Diploma/certificate from the military in Medical Specialist Course.
2 years directed patient care at MA level or greater.
