Description
The Clinical Care Coordinator provides administrative support to the Clinical Care team by managing the electronic medical records system. The Clinical Care Coordinator also supports administrative functions of the back office to include scheduling field clinicians, tracking and updating CTI's and Outside Attending Physician Agreements, uploading orders received, monitoring faxes for incoming orders or referrals, attending biweekly IDG and acting as scribe, and processing general workflow for your team.
Essential Job Functions / Responsibilities
Accurately enters a variety of data into various information systems
Executes workflow functions in electronic medical record system, Home Care Home Base (HCHB)
Verifies accuracy of data entered through collaboration with Clinical Manager(s) and/or Clinical Director
Manages incoming phone calls and directs to the appropriate team member
Provides customer support to patients and colleagues via phone, email and fax
Alerts appropriate management team members regarding potential compliance issues.
Maintains confidentiality of PHI and organization information at all times
Orders DME and alerts DME supplier when equipment is no longer in use or needs to be exchanged, etc.
Participates and scribes IDG meetings
Performs other specific projects relating to office procedures
Provides training and ongoing support to new Clinical Care Coordinators within the region
Position Qualifications
High School Diploma (College Degree Preferred)
At least two (2) years health care experience, preferably in hospice
At least five (5) years customer service experience
Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities
HCHB/Pointcare knowledge is preferred but not required
