Overview
Clinical Care Coordinator (3025) - West Point Family Medicine at Tidewater Physicians Multispecialty Group (TPMG). TPMG is comprised of over 200 physicians and advanced practice clinicians and is the largest physician-owned group on the Peninsula. This is a full-time position, Monday - Friday.
Base pay range: $19.00/hr - $25.00/hr. This range is provided by TPMG. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more.
TPMG is actively seeking a Clinical Care Coordinator for our family medicine office in West Point.
Responsibilities
Oversees clinical staff daily office operations and delegates as needed with Office Manager.
Responsible for providing overall leadership, growth, administration and performance for clinical staff to ensure accomplishment of its objectives and goals.
Room patients, triage and input vital signs for Providers.
Work with OM for clinical staff personnel functions including staffing, training, monitoring and evaluation of staff.
Develops guidelines for prioritizing work activities, evaluating effectiveness, and modifying activities as necessary.
Implementation in clinic of new service lines, Nextgen requirements, TPMG requirements for ACO, CCM and new processes.
Oversee supply ordering for clinical and durable medical equipment (DME).
Responsible for durable medical equipment (DME) and injectable inventory management systems to ensure the clinic is adequately stocked to meet patient needs.
Maintain OSHA compliance and manuals.
Maintain clinical certifications and record in Paycor.
Assisting physician with gynecologic, surgical and wound procedures/exams.
Administering immunizations and variety of injections.
Serve as a resource to all staff about patient care coordination, availability, and any additional pertinent information.
Maintains strictest confidentiality.
All other clinical duties as assigned.
Knowledge, Skills And Abilities
Knowledge of organizational policies, procedures and systems.
Knowledge of clinic office procedures.
Knowledge of computer systems and applications.
Knowledge of medical practices, terminology, and reimbursement policies.
Knowledge of grammar, spelling, punctuation and sentence structure to answer correspondence and prepare reports.
Skill in planning, organizing, delegating and supervising.
Skill in evaluating the effectiveness of existing methods and procedures.
Skill in operating a variety of office equipment and computer programs.
Ability to work scheduled hours as defined in the job offer.

Education / Training / Requirements
Associates degree or equivalent.
Minimum of 5 years related experience.
Current licensure in VA as an CMA or
LPN or Registered Nurse preferred.
CPR Certification.

Ability to sit for extended periods of time.
Ability to enter data into a computer via a keyboard.
Ability to occasionally reach, bend, stoop and lift up to 30 lbs. *
