Job Summary
The Community Bridges, Inc. (CBI) Client Care Coordinator is a shelter or temporary lodging site-based position responsible for managing a housing caseload and coordinating care for individuals and families experiencing homelessness. The role involves providing care coordination and support services to help end episodes of homelessness, especially for individuals with complex substance use, mental health, housing, and medical needs. The Coordinator ensures connection to appropriate services, monitors community-based services, and facilitates housing searches.
The position requires documentation and data entry into established systems, assisting with reports and shift end documentation. The Coordinator works within a multidisciplinary team and coordinates with community providers, tracking interactions according to relevant regulations and standards.
Skills/Requirements
High school diploma or GED required; Associate degree or higher preferred.
Minimum six months of recovery from substance use and/or mental health disorders.
Minimum six months of paid related work experience in behavioral and medical crisis situations.
1-3 years of full-time healthcare-related experience preferred.
