Job Summary
The Client Care Coordinator works in a shelter or temporary lodging setting to manage a housing caseload and coordinate care for individuals and families experiencing homelessness. The role involves coordinating medical, behavioral, and housing services, ensuring connections to PCMH/primary care, and monitoring community‑based services. The coordinator documents interactions, completes data entry, and reports on case outcomes.

Responsibilities
• Coordinate care through the CBI Continuum of Care and other community agencies, addressing social determinants of health such as housing, crisis, urgent care, and behavioral health services.

• Use housing search software to help individuals identify suitable units.

• Document interactions and track required interactions per state, federal, and CBI policies.

• Complete data entry, generate end‑of‑shift reports, and assist leadership with ongoing reports.

Qualifications

High school diploma or GED required; associate degree in a behavioral health related field preferred.

Minimum six months recovery from substance use and/or mental health disorders.

Minimum six months paid related work experience in behavioral and medical crisis situations; 1–3 years full‑time health care related work experience preferred.

Valid identification required.

Will complete CBI Peer Certification within 90 days of being hired.

Arizona Fingerprint Clearance Card (Program Specific).
