Join our mission to transform healthcare delivery from reactive, episodic care to proactively managed patient care that prevents life‑changing problems before they happen for patients with two or more chronic conditions. Every patient with chronic disease deserves consistent check‑ins, follow‑up, and support.

The Nurse Chronic Care Coordinator will perform telephonic encounters with patients on behalf of our partners each month and develop detailed care plans within our care plan templates in the EHR. This begins as an independent 1099 Contractor position but offers the potential to reach full‑time W2 employment with benefits.

Harris CCM is hiring nurses to work part‑time from home while complying with HIPAA privacy laws. You will set your own hours and are not held to a daily work hour schedule. Contracted to work a minimum of 20 hrs/week; typical workload 20‑30 hours per week. Flexibility to choose dates and hours.

The Care Coordinator will be assigned a patient panel based on skill and efficiency and is expected to manage a minimum of 100 patients per calendar month, completing encounters with at least 90% of assigned patients.

Payment: Productivity‑based pay: $10.00 per completed patient encounter up to 99 encounters/month, $10.25/encounter from 100‑149, $12/encounter from 150‑199, $14/encounter from 200‑249, $16/encounter for >250 encounters/month. Payment tier increases require 3 months consistency. A patient encounter takes a minimum 20 minutes (time is cumulative).

What Your Impact Will Be

Abide by the plan of care and orders of the practice.

Provide prevention and intervention for multiple disease conditions through motivational coaching.

Develop a positive interaction with patients on behalf of our practices.

Improve revenue by creating billable CCM episodes and increasing visits for chronic condition management.

Create detailed care plans for doctors and patients, focused on prevention and intervention.

Understand health care goals associated with chronic disease management provided by the practice.

Attend regularly scheduled meetings (bi‑monthly staff meetings, monthly one‑on‑ones, etc.) to define current scope of work.

What We Are Looking For

Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.).

Current COMPACT license to practice as an RN/LVN/LPN held in current state of residence with no disciplinary actions noted.

Minimum of two (2) years of clinical experience in Med/Surg, Case Management, and/or home health care.

Hands‑on experience with Electronic Medical Records and understanding of Windows desktop and applications (Microsoft Office 365, Teams, Excel, etc.), while working in an HIPAA‑compliant area at home.

Exercise initiative, judgment, organization, time‑management, problem‑solving, and decision‑making skills.

Skilled in using various computer programs (If you don’t love computers, you won’t love this position!).

High‑speed Internet and a desktop or laptop computer (Windows or Mac OS). No Chromebooks.

Excellent verbal, written and listening skills are a must.
