Overview The Child & Family Care Coordinator (CFCC) is responsible for providing care coordination to children with developmental disabilities and their families. The CFCC will assess individual and family needs and provide referral services and connections to necessary resources. Candidate must be familiar with the community resources available to support children and families with a wide range of identified needs (recreation, education, social, medical, etc.). This position is instrumental in developing relationships with individuals Upper Valley Services will continue to support through their adult years. A strong focus will be around development of school transition supports to ensure a smooth process as individuals graduate from high school. Additional information will be provided during the pre-screen process. It is essential all applicants be able to work within a team as well as independently. Must have valid driver’s license and pass background checks. Training for the right applicant will be available.
Responsibilities Provide care coordination for children with developmental disabilities and their families.

Valid driver’s license and ability to pass background checks.
Willingness to participate in training for the role.
