Child and Adolescent Mental Health Case Manager
Youable Emotional Health is hiring dedicated professionals to coordinate mental health case management services for children and families in the metro area. We empower families and advocate for emotional health for all ages.
Target Start Date: January 2026
The Case Management team arranges and facilitates a variety of services for children with significant mental health symptoms that impact their lives and the families served. This client‑centered role works throughout the community to understand client perspectives and is part of a strong, supportive team.
Job Responsibilities
Manage an average of 15 active clients per month.
Develop, implement, and monitor individualized case management treatment plans and functional assessments in accordance with organizational, DHS, insurance, and county guidelines.
Coordinate and facilitate a variety of services for children and families in the community.
Build strong relationships with clients and community partners.
Complete documentation and maintain accurate client records.
Job Qualifications
Bachelor’s degree in behavioral sciences or a related field from an accredited college or university.
For social workers, an LSW is required.
Experience working with families and children.
Organizational skills and the ability to complete documentation.
Strong relationship‑building skills and a desire to serve diverse populations.
Valid driver’s license and reliable auto insurance.
