The Choices for Independence (CFI) Case Manager is responsible for coordination of services for clients as well as the day‑to‑day operations of CFI services. The CFI Case Manager completes assessments, develops a care plan in conjunction with the client, coordinates services identified and documents and monitors services provided.
Ideal Candidate Attributes
Bachelor’s degree plus at least two years’ experience in the human service industry
Excellent oral and written communication skills
Proven ability to work independently and in collaboration with others with little direction
Able to maintain confidentiality at all times
Ability to utilize technology provided to perform requirements of the job
