Description Of Job
Provides daily case‑management functions for offenders, using an advocacy‑based team approach to support successful re‑entry into society.
Meets with individual offenders to guide them in accordance with CDOC regulations and federal/state laws.
Delivers case‑management services utilizing motivational interviewing and cognitive‑based interventions aligned with the Colorado Transitional Accountability Plan (CTAP).
Identifies and collects meaningful information regarding criminogenic needs.
Determines individual case plans, programmatic needs, and goals in collaboration with the offender.
Coordinates access to work, education, treatment, and recreational programs, and develops parole plans.
Monitors, evaluates, and records progress; completes assessments at regular intervals per CDOC policy.
Maintains regular interaction with offenders, coordinating medical, mental health, and treatment services.
Researches case files and prepares plans for presentation to the community and parole boards.
Assists offenders in preparing for transition.
Manages a multidisciplinary network to provide feedback, crisis intervention, and grievance resolution.
Teaches pro‑social classes to offenders.
Other duties as assigned.
Minimum Qualifications
Three (3) years of experience working directly with a hard‑to‑serve, high‑needs client population in a correctional or human services environment.
Experience including case planning, risk/needs assessment, coordination of community services, and monitoring compliance.
OR three (3) years of corrections experience that includes at least one (1) year with a hard‑to‑serve population, involving offender classification, programming, or daily living placement.
OR a combination of education (Criminal Justice, Social Work, Sociology, Psychology, or related field) and experience equal to the above.

Preferred Qualifications
Knowledge of evidence‑based practices for inmate re‑entry.
Experience coordinating services for high‑risk clients.
Proficiency in Microsoft Office and Google Suite.
Conditions of Employment
Must be 21 years of age.
Must possess a valid driver’s license.
Must have a high school diploma or GED.
Must be legally eligible to work in the United States; no sponsorship.
Must successfully complete the six‑week Basic Training program.
