Job Summary
The CCHIP Case Management (CM) will provide a range of client‑centered, confidential services that link clients with health care, clinical psychosocial, and supportive services for clients living with HIV/AIDS who are identified as having challenges with accessing and maintaining adherence to health care services. The CM works closely with the CORE Center medical team to stabilize clients medically, facilitates linkage to and maintenance of primary medical services, and provides treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments. The CM assures clients are connecting to other core services – dental, mental health, and substance abuse treatment – and offers benefits counseling, enrollment, verification, and utilization support. This case manager will also provide case management to special populations.
This is a grant‑funded position (Grant – CCH Ryan White pt B Reentry Region 8) that will expire on June 30, 2026, with the potential to be renewed.

Typical Duties
Completes initial intake and assessment of needs for new clients and returning clients.
Completes Aids Foundation of Chicago (AFC) intake documentation.
Makes all initial referrals to Behavioral Health, Health Education, Dental, and Benefits.
Assesses benefits and completes necessary applications and referrals.
Maintains a caseload based on funders’ requirements.
Develops a comprehensive, individualized service plan in collaboration with the client, including specific outcomes and timelines.
Conducts periodic re‑evaluations at least every 6 months to review goal attainment and update the plan.
Collaborates with medical providers and clinic staff to assure compliance with the service plan.
Maintains contact with clients to ensure medical and medication compliance.
Reminds clients of scheduled medical appointments.
Conducts face‑to‑face contact with clients at least once every three months and phone contact monthly in between.
Outreaches clients who become non‑compliant, using phone, letters, and home visits to encourage compliance.
Advocates for clients at medical appointments and during pre‑clinic visits.
Provides transportation assistance when needed.
Coordinates appropriate referrals and follows up to ensure completion of services.
Transitions clients out of medical case management into supportive programs or closure when goals are met.
Maintains documentation in the electronic medical record and other required databases.
Participates in CORE Center and AFC mandatory training and meetings.
Attends quality improvement efforts and provides expertise on HIV/AIDS programs.
Provides general coverage when shortages arise and facilitates groups upon request.
Minimum Qualifications
Associate’s degree in human services, social science, or health education from an accredited college or university with two (2) years of experience working with LGBTQIA and/or HIV/AIDS clients, or
Bachelor’s degree in the same field with prior experience working with LGBTQIA and/or HIV/AIDS clients.
Proficiency using Microsoft Office, including Access and similar database programs.
Must obtain and maintain an AFC Medical Case Management Certification within the first 6 months of employment.
Preferred Qualifications
Three (3) years of experience working with HIV/AIDS clients with multiple needs.
Licensed LSW or LCSW.
Experience in a primary care medical setting, hospital, outpatient setting, and/or with clients with multiple needs.
Knowledge, Skills, and Abilities
Knowledge of HIV/AIDS and family issues, commitment to confidentiality, ethics, and a culturally sensitive approach to counseling.

Physical and Environmental Demands
This position requires adherence to all hospital and department specific safety requirements, including personal protective equipment, hand washing, sanitizing practices, and engineering controls. The incumbent must also comply with safety precautions specified by hospital‑wide policy and departmental procedures.
