We are seeking an experienced Nurse Case Manager to work with injured workers and workers’ compensation cases in Rochester, NY and surrounding communities. This home‑based position requires occasional travel up to 2 hours one‑way to doctors’ offices, hospitals, and various other locations. The Case Manager will be responsible for medical case management of work‑related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. Responsibilities include working with insurance carriers, medical care providers, attorneys, employers, and employees, closely monitoring progress of the injured worker and reporting results back to employers and insurance carriers.

Qualifications

Current, unencumbered registered nurse (RN) license in NY

Experience in workers’ compensation catastrophic case management. May include critical care experience, catastrophic injury/illness case management, disability management, clinical experience in acquired brain injury, spinal cord injury, multiple trauma (including amputation), and/or burns.

Required certifications (CCM, CRRN, CDMS, CRC) within two years of hire.

Ability to occasionally work outside of traditional business hours.

Skills: organization and timeline adherence, PC and technology, communication (written, verbal, interpersonal).

Valid driver’s license and good driving history.
