Overview Under the supervision of the Youth and Family Clinical Program Manager, the case manager provides recovery-oriented case management services, skill building, and medication education. The case manager conducts routine psycho-social-emotional needs assessments, completes Person-Centered Recovery Plans, assists youth and their families with accessing community resources, provides rehabilitative services, and initiates crisis intervention when needed. All services are documented in an electronic health record and provided in collaboration with a multidisciplinary recovery team.

Responsibilities

Provide recovery-oriented case management services, skill building, and medication education under supervision of the Youth and Family Clinical Program Manager.

Conduct psycho-social-emotional needs assessments and develop Person-Centered Recovery Plans.

Assist youth and families with accessing community resources; provide rehabilitative services; initiate crisis intervention as needed.

Document all services in the electronic health record; collaborate with a multidisciplinary recovery team.

Promote recovery and wellness, community integration, independent living within the community, and improved quality of life.

Maintain a high level of detailed electronic documentation; adhere to company and state guidelines; direct service hours to meet minimum requirements (e.g., 55% of hours).

Qualifications

Bachelors degree in Human Services, or a minimum of 24 college credits in a human services field if the degree is not in Human Services.

Knowledge of mental health diagnoses and treatment; ability to work independently and collaboratively in a fast-paced, recovery-focused environment; demonstrated ability to connect with people.

Valid Texas Driver's License and maintained driving status throughout employment.

Preferred: Masters degree in Human Services; one year of experience in wellness and recovery support services in mental health, substance use, and/or crisis intervention; pursuit of clinical licensure in counseling, behavior analysis, family therapy, social work, or related field; advanced licensure; bilingual in ASL or Spanish; experience with concurrent documentation; Licensure as a Licensed Chemical Dependency Counselor (LCDC).
