GULF COAST CENTER - JOB DESCRIPTION: Job Title: Case Manager-YBHS
Location: Galveston County
FLSA Status: Non-Exempt
Working Status: Full Time with Benefits
Summary: Provide treatment through Texas Resiliency and Recovery for children and adolescents enrolled in Youth Behavioral Health Services with The Gulf Coast Center (GCC). Primary responsibility will be to coordinate and implement TRR as a member of the interdisciplinary team. Responsibilities include consultation with other team members, developing natural support options within community settings, monitoring youth's assigned level of care, and assuring all documentation is completed as required.
Job Duties: Essential Job Duties: Manage assigned caseload as part of an interdisciplinary team following program procedures.
Conduct initial screenings and provide crisis intervention and referrals as appropriate.
Conduct the Children's Needs & Strengths Assessment (CANS) every 3 months or as clinically indicated.
Ensure all necessary consent forms are completed.
Display knowledge in TRR to assure proper protocol during each intake scheduled.
Complete QMHP assessment necessary for service delivery.
Document all required information correctly and timely.
Provide skills training, parent training, and medication training as determined by state curricula.
Provide crisis services as needed.
Participate in team meetings and clinical staffings.
Required Education: Requires a baccalaureate degree or equivalent in the behavioral sciences field from an accredited institution.
Preferred Experience: One year related Mental Health experience preferred.
Work related experience with children preferred.
Abilities/Traits: General knowledge of mental disorders and child development.
Self-motivated with good time management skills.
Effective interpersonal skills.
Ability to establish cooperative relationships with youth and families.
Working Conditions: Work performed in Office, Field, and/or Remotely.
