Residential Case Manager

The Residential Case Manager is responsible for coordinating all aspects of the residents’ treatment while admitted to the residential program. This includes coordinating, linking, assisting, planning, attending treatment appointments, monitoring, mentoring, educating, teaching, developing treatment plans and work plans. The case manager will consistently assess the residents’ progress, ensure they move forward at a pace in line with the discharge date, conduct all assessments upon and during admission (functional assessment, risk, etc.), and communicate progress and concerns with staff involved in the residents’ treatment.

Essential Functions

Excellent written and oral communication skills.

Provide daily oversight to all residents in the program.

Develop ongoing transition plans targeting movement from program to community living within required length of stay.

Develop discharge planning per program standards.

Coordinate all treatment team meetings.

Complete all life‑skills assessments.

Complete quarterly treatment plans.

Prepare monthly budgets for all payee residents.

Ensure all residents have proper entitlements (Social Security, Medicaid, Medicare, etc.).

Complete crisis plans as needed.

Maintain clients’ paper/ECR charts.

Complete internal and external referrals (SMHA, medical providers, OASIS, etc.).

Connect with treatment team or resident within 24 hours of hospitalization.

Attend all treatment team meetings with inpatient staff.

Adjust acuity or MGAF scores as needed.

Monitor residents and administer medications as scheduled.

Enter daily schedule into the electronic medical record.

Complete daily documentation per program requirements.

Conduct apartment checks to ensure residents maintain cleanliness.

Assist residents in developing daily living skills (cooking, cleaning, shopping, transportation, budgeting, personal hygiene, sex education, etc.).

Report behavioral or mental state changes to appropriate team members.

Use motivational interviewing techniques to enhance recovery success.

Provide transportation to appointments or activities.

Participate in and facilitate all safety drills.

Report maintenance, environmental or emergency situations to appropriate people.

Navigate self in each resident’s electronic client record.

Manage caseload daily, including appointments, activities, mentoring, and ADL skills.

Provide direct feedback to Program Coordinator as required.

Maintain cleanliness of office as needed.

Qualifications
Associate’s degree in a related field plus at least two (2) years of behavioral health or human services experience (custodial care does not count). Current, valid driver’s license required. Competent user of technology (e‑mail, agency intranet, electronic client record). Excellent written and oral communication skills. CPR/First Aid certified or eligible.
